Intake form

To help us provide the best possible care for your equine companion, please fill out this intake form. The information gathered will allow us to tailor each session to your horse's specific needs, ensuring a thoughtful and individualized approach.

Information we need

When a new client signs up, we gather basic information about the horse, including age, breed, history, current use (performance, companion, rescue, etc.), and any known injuries or medical conditions. We also ask about behavior, temperament, and any sensitivities the horse may have.

We request recent photos and, when possible, short videos to better understand movement, posture, and overall condition. It’s also helpful to know if the horse has received prior bodywork or therapy and what the goals are—whether that’s relaxation, recovery, improved performance, or emotional regulation.

Preparing for your session

To help ensure a smooth and effective session, we ask that your horse is in a quiet, safe environment where they feel comfortable and not rushed. Please avoid heavy exercise right before the session, and make sure your horse is reasonably clean and dry.

It’s helpful if the handler remains calm and present, as horses are highly responsive to energy and environment. We encourage a relaxed atmosphere with minimal distractions so the horse can fully benefit from the experience.

Most importantly, come with an open mind—each session is guided by the horse’s comfort level, and we work at their pace to create a positive and supportive experience.

Our goal

The goal of this form is to gain a clear understanding of the horse’s physical condition, emotional state, and history, along with the client’s intentions for the session. This allows us to tailor each experience to support both horse and handler in a thoughtful and individualized way.

It also helps establish a calm, informed approach so each session is safe, effective, and aligned with the overall goals of relaxation, recovery, connection, and well-being.
